You may complete the form below to have a greeting card delivered to a patient currently admitted to Upstate Community Hospital. While your message is confidential, it is viewed by a staff member and printed into a physical card to be delivered to your patient.

This is not a confirmation that this person is a current patient or ever was a patient at Upstate Community Hospital.

Please note that e-cards are not delivered on weekends.
